Abstract
Amaç: Diz eklemi dejeneratif değişikliklerinde artroskopik debridman sonrasi eklem içi sodyum hyaluronat uygulamasının klinik sonuçlarını değerlendirmek. Çalışma planı: Artroskopik incelemede medial femoralkondilde kondral patoloji saptanan 57 hastanın 57 dizindeartroskopik debridman uygulandı. Hastaların 19'u erkek, 38'i kadındı (ort. Yaş 56; dağılım 31-75). Outerbridge sıniflamasma göre 26 dizde 1-2. derece (grup A), 31 dizde 3-4. derece (grup B) kondral lezyon saptandı. Debridmanla birlikte parsiyel menisektpmi ve plika eksizyonu uygulanan olgular çalişmaya alınmadı. Hastaların tümüne artroskopik debridman sonrasi birer hafta aralıklarla toplam üç kez eklem içi sodyum hyaluronat enjeksiyonu yapıldı. Ameliyat sonrasi değerlendirme Lysholm skorlamasma göre altıncı ve 22. aylarda yapıldı. Sonuçlar: Altıncı ayda Lysholm skoru ortalamasında grup A'da 41.5 puan, grup B'de 29.8 puan artış saptandı. Altıncı aydaki skorlara göre çok iyi ve iyi sonuçların oranı grup A' da %84.6, grup B'de %38.7 bulundu. Yirmi ikinci ayda grup A'da skorda 7 puan düşüş, grup B'de 8 puan düşüş saptandı. Çok iyi ve iyi sonuçların oranı grup A'da %69.2, grup B'de %22.5 bulundu. Çıkarımlar: Artroskopik debridman sonrasi eklem içi sodyum hyaluronat enjeksiyonunun ilk altı aylık dönemde ağrı ve fonksiyon açisindan etkili olduğu, ancak bu etkinin uzun süre devam etmediği görüldü.
Objectives: To evaluate the efficacy of intraarticular sodium hyaluronate injection following arthroscopic debridement in the treatment of degenerative knee diseases. Methods: Arthroscopic debridement was performed in 57 patients for chondral lesions detected in the medial femoral condyle. There were 19 males and 38 females (mean age 56 years; range 11-75 years). According to the Outerbridge classification, the chondral lesions were grade I-II (group A) in 26 knees and grade III-IV (group B) in 31 knees. Patients in whom debridement was performed together with partial meniscectomy and plica excision were excluded. In all cases, following arthroscopic debridement, intraarticular sodium hyaluronate injection was performed once a week for three weeks. Postoperative evaluation was made according to the Lysholm scoring system in the 6th and 22nd months. Results: At the sixth month of follow-up, the mean Lysholm scores increased by 41.5 and 29.8 points in groups A and B, respectively, with excellent-to good results being 84.6% in group A, and 38.7% in group B. At the 22nd month, the scores decreased by 7 points in group A and 8 points in group B, with excellent-to good results being 69.2% and 22.5%, respectively. Conclusions: We concluded that the efficacy of intraarticular sodium hyaluronate injection following arthroscopic debridement in relieving pain and functional improvement did not last long.
